HOW TO REGISTER

Our area extends from School Lane, Broomfield in the north to Springfield Road in the south and Chignal St. James in the west to Pump Lane in the east. If you are unsure whether or not you live in our catchment area, please telephone the surgery and we will advise you.

If you are not in our area please telephone Essex Contractor Services on 01255-204000 and they will advise you of your nearest doctors’ surgery.

We need to have proof by way of a Utility bill or bank statement etc of your name and address before we can register you.
If you have a medical card –

• Sign the card where indicated
• Enter your new address and telephone number
• Bring the card to the surgery
• Complete a health questionnaire

If you do not have a medical card –

• Come to the surgery and complete a registration form
• Complete a health questionnaire

If you are 50 years of age or over and/or on regular medication, you will be asked to make an appointment with the Health Care Assistant or Nurse for a new patient check. When you attend this appointment please bring with you a urine sample and a list of your regular medication.
