PRESCRIPTIONS & TEST RESULTS

Repeat Prescription Requests

To order a repeat prescription, tick the items you require on the right-hand page of your last prescription and take or post it to the surgery. If you do not have the right-hand page of your last prescription, simply write a note of what you require, remembering to include your full name and address. If you would like your prescription posted back to you, please enclose a stamped addressed envelope.

We regret that we are unable to take prescription requests over the telephone. However they can be faxed to us on 01245-344287.

PLEASE ALLOW 48 HOURS FOR YOUR PRESCRIPTION TO BE READY

Several local chemists operate a system whereby you can leave a repeat request with them, and collect your medication without the need to visit the surgery. Please contact the chemist directly for further details. Local chemists who currently offer this service are:

Melbourne Pharmacy, Melbourne Avenue 01245-355645
Alliance Pharmacy, Sunrise Avenue 01245-281350
Moss Pharmacy, Somerfield Supermarket 01245-443317
Boots, High Chelmer Shopping Precinct 01245-258252
Shadforth Pharmacy, Broomfield Parade 01245-251496

Medication Name Changes

Due to European Law, with effect from 1 September 2006, the names of some medications will change. This is only a name change. There is no change to your medication.
